Wayne Memorial Community Health Centers (WMCHC) offers primary care services across nineteen sites. We have offices located in Honesdale, Hamlin, Carbondale, Forest City, Waymart, Tafton, Lords Valley and Lake Como. Our staff of physicians, advanced practice providers and support professionals work as a team to provide patients with the most coordinated and responsive care possible. Services include routine exams and immunizations, treatment and diagnosis of acute conditions, management of chronic diseases and referral to specialists as needed.  

Wayne Memorial Community Health Centers is a progressive multi-specialty group made up of primary care, women’s health, pediatrics, behavioral health, dental and podiatric offices in northeastern Pennsylvania. We have 19 sites serving nine communities across four counties. Our organization employs over 400 individuals including physicians, dentists, nurse practitioners, physician assistants, certified nurse midwives, licensed behavioral health therapists, administrative and office support staff. WMCHC is clinically affiliated with Wayne Memorial Health System, Inc.
